- Accredited Home Care - Accredited Home Care will participate in the National Day of Action by sharing information and resources via social media.
- Affinity Medical Group (WI) - Affinity Medical Group will distribute the Circulation Nation tip sheets, developed by Measure Up/Pressure Down®, throughout National High Blood Pressure Education Month.
- Allegan County Building Ties (MI) - Allegan County Building Ties will host free blood pressure screenings and education at its Allegan County Senior Fest 2015. Pharmacists will also be on hand to answer questions about medications.
- American Diabetes Association's Buffalo Office (NY) - American Diabetes Association's Buffalo Office will participate in the National Day of Action by sharing information and resources via social media.
- American Diabetes Association National Capital Area - ADA National Capital Area will be participating in the Roll Up Your Sleeves! Health Expo on Saturday, May 9 in Washington, DC, presented by Measure Up/Pressure Down® in partnership with United Health Foundation, YMCA of Metropolitan Washington, and District of Columbia Department of Health. During the event, ADA National Capital Chapter will be educating attendees about high blood pressure and diabetes.
- American Heart Association - The American Heart Association will participate in the National Day of Action by including the event in its own High Blood Pressure Education Month Toolkit and sharing information on its Check. Change. Control. Leadership Community and social media.
- American Heart Association Alabama Chapter (AL) - American Heart Association Alabama Chapter will participate in the National Day of Action by sharing information and resources via social media.
- American Heart Association Greater Washington Region (DC/VA/MD) - The American Heart Association Greater Washington Region will be participating in the Roll Up Your Sleeves! Health Expo on Saturday, May 9 in Washington, DC, presented by Measure Up/Pressure Down® in partnership with United Health Foundation, YMCA of Metropolitan Washington, and District of Columbia Department of Health. During the event, AHA will be educating attendees about heart health and high blood pressure.
- American Journal of Managed Care - American Journal of Managed Care will participate in the National Day of Action by sharing information and resources via social media.
- "American Kidney Fund - Campaign partner American Kidney Fund will share information about high blood pressure on social media and its blog, as well as highlight blood pressure control in its May e-newsletter. In addition to these action plans, AKF will be participating in the Roll Up Your Sleeves! Health Expo on Saturday, May 9 in Washington, DC, presented by Measure Up/Pressure Down® in partnership with United Health Foundation, YMCA of Metropolitan Washington, and District of Columbia Department of Health. During the event, the American Kidney Fund will be educating attendees about high blood pressure and its connection to kidney disease."
- American Medical Association: As part of its National Day of Action: Roll Up Your Sleeves! efforts, the American Medical Association will share information about the event via email with stakeholders, including pilot sites, Hypertension Control Advisory Group members, spread partners, and patient and family advisors. In addition, AMA will also publish weekly AMA Wire stories about hypertension control, feature tools on its website to improve hypertension control, offer an employee blood pressure screening event, host an AMA wellness walk, and participate in a Twitter chat.
- American Medical Group Association (VA) - The American Medical Group Association will offer a free heart-healthy lunch to employees and view the new campaign "It Takes a Team" and "Choose Your Path" videos.
- American Medical Group Foundation (VA) - The American Medical Group Foundation will offer a free heart-healthy lunch to employees and view the new campaign "It Takes a Team" and "Choose Your Path" videos.
- American Society of Hypertension - The American Society of Hypertension will be participating in the Roll Up Your Sleeves! Health Expo on Saturday, May 9 in Washington, DC, presented by Measure Up/Pressure Down® in partnership with United Health Foundation, YMCA of Metropolitan Washington, and District of Columbia Department of Health. During the event, ASH will be educating attendees about high blood pressure.
- Anceta, LLC (VA) - Anceta, LLC will offer a free heart-healthy lunch to employees and view the new campaign "It Takes a Team" and "Choose Your Path" videos.
- Arch Health Partners (CA) - Arch Health Partners will take three "actions" for the National Day of Action: 1) Ambulatory care pharmacy team will see patients and treat to goal using Arch Health Partners' in-house treatment algorithm. 2) Display posters and signage in all building lobbies with the caption "Who wants to be AVERAGE?" 3) Offer free blood pressure screenings and promote its Measure Up/Pressure Down® campaign involvement at an area street fair on May 2.
- AtCor Medical - AtCor Medical will promote the National Day of Action through its social media channels.
- Atrius Health (MA) - Atrius Health will promote the National Day of Action through its social media channels.
- Austin Regional Clinic (TX) - Austin Regional Clinic will conduct outreach to patients with high blood pressure to schedule follow-up appointments. In addition, an article about the importance of recording and managing blood pressure in the electronic medical record will be shared with all employees, and social media posts will be targeted to patients.
- Baptist Medical Group (TN) – Baptist Medical Group will launch targeted outreach to patients with uncontrolled hypertension as part of their efforts for National High Blood Pressure Education Month in May and beyond. This initiative will help thousands of patients return to the office for blood pressure checks and hypertension treatment. And, for the second consecutive year, the Memphis-based organization is asking its employees to get their blood pressure checked on May 7 and record their reading in a competition with other clinics.
- Baylor Health Care System/HealthTexas Provider Network (TX) - Baylor Health Care System/HealthTexas Provider Network will offer an in-service to staff about the Do's and Don'ts of Taking a Blood Pressure (to ensure accurate blood pressure measurement, campaign plank #1). The organization will also reach patients about blood pressure control through its website.
- Bend Memorial Clinic (OR) - Bend Memorial Clinic will be raising awareness of high blood pressure through social media posts and an article in its May newsletter.
- Billings Clinic (MT) - Billings Clinic will be raising awareness of high blood pressure through social media posts.
- BlueCare Tennessee (TN) - BlueCare Tennessee will promote the National Day of Action and blood pressure management to its members and employees. To reach its members, BlueCare Tennessee will post Measure Up/Pressure Down® content on Facebook and website, identify community events scheduled for May 7, and incorporate a call to action around high blood pressure in all outbound call scripts for the day. For its employees, all staff will be encouraged to wear red on May 7, offered blood pressure screenings onsite in breakrooms as well as at regional sites, requested to educate members at the end of each inbound telephone call about the National Day of Action (the staff that has the highest number of touches will receive a prize), and display educational posters in the office. Patient educational materials will be distributed both in the office to employees and at community events to members.
- BodyCraft Fitness - BodyCraft Fitness will share high blood pressure information and resources with social media followers.
- Brown & Toland Physicians (CA) - Brown & Toland Physicians will increase awareness among consumers about the importance of taking your blood pressure on a regular basis, knowing your numbers, and the risks of hypertension if your blood pressure is left untreated. To do so, Brown & Toland will host a free blood pressure screening event for the public on the National Day of Action. Nurses will be on hand in San Francisco (2100 Webster Street from 10am-2pm) and East Bay (North Berkeley Farmers Market from 3-7pm) to take blood pressure screenings and provide the results (without medical analysis). In addition, Brown & Toland will extend their promotion through October by increasing physician awareness of the importance of correctly measuring, recording, and managing blood pressure, through information in all physician-facing publications as well as handouts and education at all physician-facing company events. Patient engagement will also be a focus, with articles in upcoming HealthLink magazines, an article on the importance of measuring blood pressure on the homepage carousel, and promotion of the events on the BTNet intranet.
- Cardiopulmonary Rehabilitation at Southern New Hampshire Medical Center (NH) - The Cardiopulmonary Rehabilitation Department at Southern New Hampshire Health Systems in Nashua, NH will be taking part in the National Day of Action. Educational materials about blood pressure, in the form of handouts, will be available to staff, patients, and the general public in common areas of the main hospital and satellite campus. Poster exhibits will also be displayed in these areas. Blood pressure screenings will be available for staff and public. Information will also be disseminated via the hospital’s video boards, intranet, and social media via the hospital Facebook page.
- Caribbean Cardiac (The Caribbean) - Caribbean Cardiac will promote the National Day of Action through its social media channels.
- Carle Physician Group (IL) – Carle Physician Group will provide free blood pressure screenings at its local mall on May 7 with on-site physician, nurse practitioner, and pharmacist to assist and educate community members. The Illinois-based medical group will also participate in a similar event, Healthy Hearts, in April. Nearly 50,000 patients with receive communications about the screening events. In addition, CPG will post information about high blood pressure on May 7 to its social media channels as well as work with local media to educate others about the “silent killer.” Lastly, the organization will review its data and care processes to continue to improve hypertension control rates.
- CHANGE, Inc. - CHANGE, Inc. will participate in the National Day of Action by sharing information and resources via social media.
- Choice Medicine - Choice Medicine will participate in the National Day of Action by sharing information and resources via social media.
- Colorado Springs Health Partners (CO) - Colorado Springs Health Partners will offer free blood pressure screenings in celebration of the National Day of Action, including an employee wellness fair and Air Force Academy event. Educational materials will also be disseminated. In addition, CSHP will share social media posts related to high blood pressure and include the National Day of Action in its employee newsletter.
- DC Fire & EMS - DC Fire & EMS will be participating in the Roll Up Your Sleeves! Health Expo on Saturday, May 9 in Washington, DC, presented by Measure Up/Pressure Down® in partnership with United Health Foundation, YMCA of Metropolitan Washington, and District of Columbia Department of Health. During the event, DC Fire & EMS will be offering free blood pressure screenings, in conjunction with MedStar Family Choice, to all attendees.
- DC Health Link - DC Health Link will be participating in the Roll Up Your Sleeves! Health Expo on Saturday, May 9 in Washington, DC, presented by Measure Up/Pressure Down® in partnership with United Health Foundation, YMCA of Metropolitan Washington, and District of Columbia Department of Health. During the event, DC Health Link will be educating attendees about high blood pressure.
- Defeat Diabetes Foundation - Defeat Diabetes Foundation will participate in the National Day of Action by sharing information and resources via social media.
- District of Columbia Department of Health (DC) - The District of Columbia Department of Health will be participating in the Roll Up Your Sleeves! Health Expo on Saturday, May 9 in Washington, DC, presented by Measure Up/Pressure Down® in partnership with United Health Foundation, YMCA of Metropolitan Washington, and District of Columbia Department of Health. The DC DOH also provided funding for the event.
- Dodge Communications (GA) - Dodge Communications will host a "BYOHL" (Bring Your Own Healthy Lunch) in commemoration of the National Day of Action, which will feature blood pressure education and free screenings. Dodge will also feature a blog post about Measure Up/Pressure Down® on May 7.
- Employer Solutions (WI) - Employer Solutions will distribute the Circulation Nation tip sheets, developed by Measure Up/Pressure Down®, throughout National High Blood Pressure Education Month.
- Enduring Hearts - Enduring Hearts will promote the Measure Up/Pressure Down® "Choose Your Path" patient video through its Facebook page.
- Erie Family Health Center-Helping Hands (IL) - Erie Family Health Center-Helping Hands will celebrate the National Day of Action with blood pressure screenings for patients and community members, as well as blood pressure education and healthy snacks.
- Esse Health (MO) - Esse Health will host a contest for direct care employees to showcase their smarts by completing a blood pressure measurement quiz. The office with the most participants and highest percentage correct will receive a certificate of achievement. In addition, Esse Health will recognize the offices that have already achieved the campaign goal of 80% blood pressure control. These plaques will be hand-delivered by Dr. Filippo Ferrigni and Esse Health's CEO on May 7, with photos to commemorate the occasion. These photos will be included in the patient enewsletter and shared on social media channels.
- Essentia Health (MN) - Essentia Health will distribute extra patient resources for clinic to make available in waiting areas the entire month of May. In addition, Essentia Health will disseminate a 10 question blood pressure IQ quick to employees. Every employee that completes a quiz will be entered into a giveaway for a gift card, with winners selected from all three Essentia Health regions.
- Evans Blount Healthcare (NC) - Evans Blount Healthcare is offering free blood pressure screenings at its Greensboro location on May 7 in celebration of the National Day of Action.
- The Everett Clinic (WA) - The Everett Clinic will promote the National Day of Action through its social media channels.
- Express Scripts - Express Scripts will participate in the National Day of Action by sharing information and resources via social media.
- Fatherhood.gov - Fatherhood.gov will participate in the National Day of Action by sharing information and resources via social media.
- Florida Medical Clinic (FL) - Florida Medical Clinic will share the Measure Up/Pressure Down® "It Takes a Team" provider/staff video and "Choose Your Path" patient video on its intranet and website.
- Greater Washington Hispanic Chamber of Commerce - The Greater Washington Hispanic Chamber of Commerce will be participating in the Roll Up Your Sleeves! Health Expo on Saturday, May 9 in Washington, DC, presented by Measure Up/Pressure Down® in partnership with United Health Foundation, YMCA of Metropolitan Washington, and District of Columbia Department of Health. During the event, Greater Washington Hispanic Chamber of Commerce will be educating attendees about high blood pressure.
- Group Health Cooperative of South Central Wisconsin (WI) - Group Health Cooperative of South Central Wisconsin will celebrate the National Day of Action and National High Blood Pressure Education Month during May. Activities will include website homepage content on high blood pressure, a post about high blood pressure from Medical Director for Quality and Care Improvement Jeff Huebner, social media posts, and a spotlight story on a patient's blood pressure success.
- Hager Sharp (DC) - Hager Sharp's Washington, DC office will share information about high blood pressure and the National Day of Action via its social media platforms.
- Harvard Vanguard Medical Associates (MA) - Harvard Vanguard Medical Associates will promote the National Day of Action through its social media channels.
- Healthcare Financial Management Association South Carolina - Healthcare Financial Management Association South Carolina will participate in the National Day of Action by sharing information and resources via social media.
- HealthyWomen - HealthyWomen will be participating in the Roll Up Your Sleeves! Health Expo on Saturday, May 9 in Washington, DC, presented by Measure Up/Pressure Down® in partnership with United Health Foundation, YMCA of Metropolitan Washington, and District of Columbia Department of Health. During the event, HealthyWomen will be educating attendees about high blood pressure and related health topics, as they impact women and their loved ones.
- Heffelfinger & Associates - Heffelfinger & Associates will participate in the National Day of Action by sharing information and resources via social media.
- Henry Ford Medical Group (MI) - Henry Ford Medical Group will launch a new video it created on proper blood pressure measurement techniques, featuring CEO Dr. William A. Conway, CEO and COO Robert G. Riney. The video will be advertised in HFMG's daily electronic company newsletter, “The Morning Post” with a link to the video on May 7. The video combines humor with the serious steps of taking a proper blood pressure. The take-home message is to promote and reinforce the correct steps in taking a blood pressure and ramifications of incorrect technique.
- Home Health Quality Improvement National Campaign - Home Health Quality Improvement National Campaign will feature the Measure Up/Pressure Down® campaign and high blood pressure on its blog during National High Blood Pressure Education Month. The National Day of Action will also be featured in the HHQI Inside Edition email newsletter.
- Innovative Health Services - Innovative Health Services will promote the National Day of Action through its social media channels.
- Innovative Health Solutions - Innovative Health Solutions will participate in the National Day of Action by sharing information and resources via social media.
- INOVA Health System - INOVA Health System will be participating in the Roll Up Your Sleeves! Health Expo on Saturday, May 9 in Washington, DC, presented by Measure Up/Pressure Down® in partnership with United Health Foundation, YMCA of Metropolitan Washington, and District of Columbia Department of Health. During the event, INOVA Health System will be educating attendees about high blood pressure.
- INOVA Medical Group (VA) - INOVA Medical Group will participate extensively in National High Blood Pressure Education Month, including the National Day of Action on May 7. Planned activities include: letters to patients about high blood pressure, social media updates, flyers, free blood pressure checks for patients and employees at each hospital location, dissemination of educational materials and giveaways, Ask the Expert hypertension seminars in two regions, press release dissemination, certificates to patients, and the "Choose Your Path" campaign video displayed in all waiting areas.
- Intermountain Health (UT) - Intermountain Health will promote National High Blood Pressure Education Month throughout May. Activities will include articles about high blood pressure every Monday, text message from Live Well regarding blood pressure the first week of May, dissemination of new "Roll Up Your Sleeves" video, and blog post about high blood pressure. In addition, Intermountain Health will acknowledge the support and role of the entire care team through letters sent to every OO, AOD, RNC, Asst. RNC, and MD within the medical group; emails sent to every care manager in primary care outpatient settings; emails sent to all cardiologists through the cardiovascular clinical program; promotion of the Measure Up/Pressure Down® "It Takes a Team" video; screen savers on all Intermountain computers the first week of May about blood pressure control; and a homepage update on the web.
- The Iowa Clinic - The Iowa Clinic will participate in the National Day of Action by sharing information and resources via social media.
- JCF Corp. - JCF Corp. will participate in the National Day of Action by sharing information and resources via social media.
- Jefferson Healthcare (WA) - As part of Jefferson Healthcare's ’s ongoing educational series WellHearts, the organization will offer a free heart health presentation, “The Silent Killer: Elevated Blood Pressure and the Risk on Your Heart,” at 6 p.m., Thursday, May 7 at the Jefferson County Library, 620 Cedar Ave., Port Hadlock, WA. After the program, free blood pressure checks will be available for those interested. The WellHearts series focuses on raising awareness, especially in women, of the importance of heart health in Jefferson County.
- Johns Hopkins Center to Eliminate Cardiovascular Health Disparities - Johns Hopkins Center to Eliminate Cardiovascular Health Disparities will participate in the National Day of Action through social media.
- Johns Hopkins Medicine Armstrong Institute for Patient Safety and Quality - Johns Hopkins Medicine Armstrong Institute for Patient Safety and Quality will participate in the National Day of Action through social media.
- Maine Center for Disease Control & Prevention (ME) - Maine Center for Disease Control & Prevention will promote the National Day of Action through social media.
- Mango Health - Mango Health will participate in the National Day of Action by sharing information and resources via social media.
- Marshall B. Ketchum University - Marshall B. Ketchum University will participate in the National Day of Action by sharing information and resources via social media.
- Mayo Health Systems (MN) - Mayo Health Systems will provide free blood pressure screenings and health information to attendees of the "First Thursday Fun Fest" in Cannon Falls, MN on the National Day of Action.
- McDonald's Restaurants of the Tri-State Area (WV/KY/OH) - McDonald's Restaurants of the Tri-State Area will offer free blood pressure screenings for customers in the greater Parkersburg, WV area on May 7, in conjunction with Mid-Ohio Valley Health Department and AmeriCorps volunteers.
- Measure Up/Pressure Down® (VA) - The American Medical Group Association will offer a free heart-healthy lunch to employees and view the new campaign "It Takes a Team" and "Choose Your Path" videos.
- MedStar Family Choice - MedStar Family Choice will be participating in the Roll Up Your Sleeves! Health Expo on Saturday, May 9 in Washington, DC, presented by Measure Up/Pressure Down® in partnership with United Health Foundation, YMCA of Metropolitan Washington, and District of Columbia Department of Health. During the event, MedStar Family Choice will be offering free blood pressure screenings, in conjunction with DC Fire & EMS, and will be educating attendees about high blood pressure.
- MedStar Franklin Square Medical Center Department of Family Medicine (MD) - The MedStar Franklin Square Medical Center Department of Family Medicine will participate in the National Day of Action by sharing high blood pressure information via the MedStar's Round the Square Stories. This will include a success story of its home blood pressure monitoring program. In addition, MedStar will promote the campaign on its Facebook and LinkedIn pages.
- Men's Health Network - Men's Health Network will be participating in the Roll Up Your Sleeves! Health Expo on Saturday, May 9 in Washington, DC, presented by Measure Up/Pressure Down® in partnership with United Health Foundation, YMCA of Metropolitan Washington, and District of Columbia Department of Health. During the event, Men's Health Network will be educating attendees about high blood pressure and related health topics, as they impact men and their loved ones.
- Mercy Clinics East Division (MO) - Mercy Clinics East Division will be publishing articles in its provider and coworker newsletters that emphasize May as National High Blood Pressure Education Month and May 7 as the National Day of Action. These will include a link to the Measure Up/Pressure Down® "It Takes a Team" video. Mercy Clinics East Division will also provide information and statistics about high blood pressure, along with Mercy's efforts over the past three years with detection and control. A healthy lunch special in the cafeterias will also be offered, along with published newspaper articles in the community and social media posts on May 7.
- MI Express Care - MI Express Care will participate in the National Day of Action by sharing information and resources via social media.
- Million Hearts® - Million Hearts® will be participating in the Roll Up Your Sleeves! Health Expo on Saturday, May 9 in Washington, DC, presented by Measure Up/Pressure Down® in partnership with United Health Foundation, YMCA of Metropolitan Washington, and District of Columbia Department of Health. During the event, Million Hearts will be educating attendees about the ABCS (including blood pressure control). In addition, feature article on CDC.gov will focus on high blood pressure and the National Day of Action. CDC and Million Hearts will also share information about blood pressure control via the website, social media channels, Twitter chats, and webinars.
- Million Hearts® Delaware - Million Hearts® Delaware will be participating in the National Day of Action by raising awareness of high blood pressure.
- Ministry Medical Group (WI) - Ministry Medical Group will distribute the Circulation Nation tip sheets, developed by Measure Up/Pressure Down®, throughout National High Blood Pressure Education Month.
- Nantucket (MA) Health Department - Nantucket Health Department will participate in the National Day of Action by sharing information and resources via social media.
- National Organization of Nurses with Disabilities - National Organization of Nurses with Disabilities will participate in the National Day of Action by sharing information and resources via social media.
- NEA Baptist Clinic (AR) - NEA Baptist Clinic will offer activities for employees as well as patients and the general public on the National Day of Action. For employees, NEA Baptist Clinic will offer free blood pressure screenings. Every employee who participates will receive a sticker to wear and be entered into a drawing for a FitBit, a facial, and a Kindle Fire. Departments with a minimum of 80% compliance will be entered into a drawing for a free lunch. In addition, the event information and photos will be posted on Clinic News, intranet, wellness newsletter, and social media. Staff will also be encouraged to walk a mile route around the campus as well as view a blood pressure screening training video. To reach patients and the general public, NEA Baptist will offer screenings in its lobby from 9-11am, with giveaways including portable blood pressure monitors, DASH Diet books, and stickers. Educational materials and blood pressure tracking cards will also be distributed. All patients with appointments will receive a blood pressure card on May 7 to record their readings. In addition, social media posts, website badges, signage, an Ask the Expert column in the local newspaper, and media pitching will further raise awareness.
- New Jersey Medical School Alumni Association - New Jersey Medical School Alumni Association will participate in the National Day of Action by sharing information and resources via social media.
- New West Physicians (CO) - New West Physicians will be disseminating the Circulation Nation tip sheets, developed by Measure Up/Pressure Down®, to all 17 office sites for display in the lobby and exam rooms. New West Physicians will also share information about blood pressure and the campaign on Facebook and Google+.
- Nothing But the Best - Nothing But the Best will promote the National Day of Action through its social media channels.
- Olmsted Medical Center (MN) - Olmsted Medical Center will provide free blood pressure screenings and health information to attendees of the "First Thursday Fun Fest" in Cannon Falls, MN on the National Day of Action.
- Palo Alto Medical Foundation - Palo Alto Medical Foundation will participate in the National Day of Action by sharing information and resources via social media.
- The Polyclinic (WA) - The Polyclinic will post the "Six Steps to Control Your Blood Pressure" article to its intranet on May 7 to all physicians and staff; share blood pressure information on its social media channels; and adopt Plank 4: All Patients Not at Goal and With New Rx Seen Within 30 Days."
- Premier Medical Associates (PA) - Premier Medical Associates will participate in this year's National Day of Action to improve high blood pressure awareness, detection, and control. Efforts will include: all employees wearing Measure Up/Pressure Down® buttons, displaying signs in waiting areas of all offices, posting photos to PMA's website, disseminating a press release to local newspapers about Premier's participation with the Measure Up/Pressure Down® campaign and its National Day of Action, and conducting outreach to patients to offer blood pressure rechecks.
- Prevea Health (WI) - Prevea Health will be raising awareness of hypertension with stickers for patients and staff that read: "The Pressure's On. Ask Me Why." In addition, the Wisconsin-based health system will provide printed information in patient waiting areas and posting a blood pressure video to its Facebook and YouTube social media channels.
- Preventive Cardiovascular Nurses Association - Campaign partner PCNA will be sharing blood pressure resources via social media. The association will also showcase a news clip of a chapter's recent blood pressure screening.
- PriMED Physicians (OH) - PriMED Physicians will educate all office staff regarding the appropriate technique to monitor a blood pressure, using Plank 1 (direct care staff trained in accurate BP measurement) as the foundation. Educational information will be disseminated to office staff with a mandatory quiz. In addition, competency checks are planned as part of the employees' annual review and will reinforce the material provided during the National Day of Action.

- Providence Hospital Fort Lincoln Family Medicine Center (MD) - Providence Hospital Fort Lincoln Family Medicine Center will participate in the National Day of Action through staff and patient education. For staff, PHS FLFMC will educate on hypertension using Measure Up/Pressure Down® resources, including for nurses, social work, front office, medical records, and community health educators. To educate patients, materials will be placed and disseminated in the lobby waiting areas as well as in-office visits.
- Quincy Medical Group (IL) - Quincy Medical Group will offer three free blood pressure screenings across the city on May 7, including free walk-in blood pressure checks at the clinic and free blood pressure checks at the State Street Bank and City of Quincy Central Services. Blood pressure checks will also be offered to QMG employees in the employee lounge on the National Day of Action, and rural sites will be encouraged to do the same. To raise awareness of high blood pressure and QMG's involvement in Measure Up/Pressure Down®, the IL-based medical group will disseminate a press release, send a patient e-blast, post an on-hold phone message, share updates on social media, and highlight the event in the May wellness e-Newsletter. All locations will also have educational materials and Measure Up/Pressure Down® stickers.
- Ralphs - All Ralphs Pharmacies will offer free blood pressure screenings to patrons of Ralphs Grocery Stores, on May 7, whether they are pharmacy customers or not. Ralphs' Pharmacists will provide counseling on hypertension, diet, fitness/exercise, and lifestyle choices. Pharmacists will attempt to provide screening results to patient's healthcare providers in order to ensure that all members of the healthcare team are aware of results (upon approved patient consent).
- Redwood Community Health Coalition (CA) - Redwood Community Health Coalition will share Measure Up/Pressure Down® resources and communicate the importance of raising awareness about high blood pressure through an newsletter and email to member community health centers on May 7. The anticipated audience includes community health center providers and staff in Sonoma, Marin, Napa, and Yolo counties.

- Right Care Initiative (CA) - The Right Care Initiative will be sharing information about the National Day of Action and high blood pressure on its website on May 7.
- Sacred Heart Hospital Pensacola (FL) - Sacred Heart Hospital Pensacola (FL) will host a regional stroke awareness night on the National Day of Action. High blood pressure is a leading risk factor for stroke. Blood pressure screenings will be offered, along with a complimentary dinner and seminar. Speakers will talk about the advancements in treating stroke and the importance of knowing the signs and symptoms when they occur.
- Santa Clara County (CA) Public Health - Santa Clara County Public Health will participate in the National Day of Action by sharing information and resources via social media.
- Sharp Rees-Stealy Medical Group (CA) - Sharp Rees-Stealy Medical Group will offer blood pressure screenings in the medical group and community as part of its National Day of Action efforts.
- Slocum-Dickson Medical Group PLLC - Slocum-Dickson Medical Group PLLC will participate in the National Day of Action by sharing information and resources via social media.
- Somerset County Department of Health (NJ) - Somerset County Department of Health will participate in the National Day of Action by sharing information and resources via social media.
- Southern New Hampshire Health System (NH) - Southern New Hampshire Health System will promote the National Day of Action through its social media channels.
- Springfield Clinic (IL) - Springfield Clinic will share information about high blood pressure and Measure Up/Pressure Down® on social media on May 7. In addition, Springfield Clinic will issue a press release and post it to SpringfieldClinic.com. The Measure Up/Pressure Down® "It Takes a Team" video will also be disseminated to staff.

- Summit Medical Group (NJ) - Summit Medical Group will celebrate the National Day of Action on May 7 by featuring a story about the campaign and efforts to date on the employee intranet, presenting certificates of achievement to providers and pods already at goal, and featuring a patient education video through social media channels.
- SwedishAmerican Health System (IL) - SwedishAmerican Health System will showcase the Measure Up/Pressure Down® "It Takes a Team" video to the quality team, care coordinators, and other care team members during National High Blood Pressure Education Month.
- ThedaCare Physicians (WI) - ThedaCare Physicians will share messages about high blood pressure and Measure Up/Pressure Down® via its Twitter and Facebook social media channels. ThedaCare will also host a site visit by the Wisconsin Collaborative for Healthcare Quality around their hypertension achievements.
- Tulane University Medical Group (LA) - Tulane University Medical Group will conduct patient outreach to schedule blood pressure screenings for patients not at goal and send out health notifications via the patient portal reminding patients to get screened. In addition, to raise awareness of the disease, TUMG will post hypertension signage in clinic areas, outfit staff with red ribbons to initiate patient conversation, and post web banners.
- United Health Foundation - United Health Foundation will be participating in the Roll Up Your Sleeves! Health Expo on Saturday, May 9 in Washington, DC, presented by Measure Up/Pressure Down® in partnership with United Health Foundation, YMCA of Metropolitan Washington, and District of Columbia Department of Health. The United Health Foundation provided funding for this event, as well as other grant activities including videos and factsheets.
- United Way of Monroe County (MI) - United Way of Monroe County will offer free blood pressure screenings on the National Day of Action at the Our Lady of Mount Carmel Catholic Church in Temperance, MI.
- Unity Health Care (DC) - Unity Health Care will adopt a new plank as part of the National Day of Action, Plank #4: all patients not at goal and with new Rx seen within 30 days.
- UnityPoint Clinic (IA) - UnityPoint Clinic will create a poster for display in exam rooms highlighting proper blood pressure measurement, encourage clinic sites to discuss proper blood pressure measurement at staff meetings and huddles, and share a link to the Measure Up/Pressure Down® video on its internal intranet.
- University of Maryland School of Pharmacy's APhA-ASP Operation Heart - The University of Maryland School of Pharmacy's APhA-ASP Operation Heart is a student-run organization that focuses on promoting cardiovascular health and awareness. For the National Day of Action on May 7, the University of Maryland School of Pharmacy's APhA-ASP Operation Heart will launch a Facebook campaign and encourage fellow students and friends to change their profile picture with a caption about high blood pressure and cardiovascular health.
- University of Utah Healthcare Community Clinics (UT) - University of Utah Healthcare Community Clinics will partake in this year's National Day of Action by promoting the campaign on video in clinic reception areas at all 10 sites; distributing hypertension infographics at all clinics; and social media posts on Twitter and Facebook.
- US Department of Health and Human Services, Region II Office of the Assistant Secretary for Health - US Department of Health and Human Services, Region II Office of the Assistant Secretary for Health will offer the Measure Up/Pressure Down® Circulation Nation tip sheets during the National Day of Action and National High Blood Pressure Education Month. In addition, U.S. Department of Health and Human Services Region II will send out a special newsletter on May 7 in recognition of the National Day of Action, as well as tweets on @HHS_OASHRegion2.
- USMD (TX) - USMD will conduct outreach to patients with elevated blood pressure readings that are not currently scheduled for an appointment. These patients will be offered the opportunity to visit a free blood pressure clinic on May 7. Outreach will be conducted via its secure email service and phone calls.
- Utah Hospital Association - Utah Hospital Association will participate in the National Day of Action by sharing information and resources via social media.
- VNA Care Network & Hospice (MA) - VNA Care Network & Hospice will promote the National Day of Action through its social media channels.

- Watson Clinic (FL) - Watson Clinic will showcase Measure Up/Pressure Down with displays at five locations during the month of May. On May 7, Watson Clinic's on-site cafeteria will feature heart-healthy menu choices.
- Weill Cornell Physician Organization (NY) - Weill Cornell Physician Organization will partner with a nearby gym and offer free blood pressure screenings in its lobby on May 7. The gym will offer discounted memberships with WCPO patients, which can incentivize patients to exercise more.
- Welch Allyn - Welch Allyn will participate in the National Day of Action by sharing information and resources via social media.
- Well3 Pikes Peak - Well3 Pikes Peak will participate in the National Day of Action by sharing information and resources via social media.
- Wellmont Health System (TN) - As part of its National Day of Action efforts, Wellmont Health System will offer free high blood pressure education materials and information to patients with elevated blood pressures, play a video from WMA within the office and hospital waiting rooms promoting the importance of knowing your blood pressure and the steps you can take to manage it, and disseminate a press release and social media posts on May 7. Wellmont Health System will also send internal employee reminders with hypertension statistics and asking "Do You Know Your Numbers" as well as sharing the same message on external electronic billboards.
- WESTMED Medical Group (NY) - WESTMED Medical Group will be hosting a patient education forum, called "140/90 More or Less." The forum will include free blood pressure screenings for all attendees, and is being promoted via web and social media.
- Withings - Withings, a campaign pilot project sponsor, will participate in the National Day of Action through educational blog posts on its website and offering screenings to employees in its Boston office building.
- WomenHeart: the National Coalition for Women with Heart Disease - WomenHeart will host a brown bag lunch for employees and discuss signs and symptoms of high blood pressure. Free blood pressure screenings will also be offered to all staff. WomenHeart will also spread the word about social media, as well as participate in the Roll Up Your Sleeves! Health Expo on Saturday, May 9 in Washington, DC, presented by Measure Up/Pressure Down® in partnership with United Health Foundation, YMCA of Metropolitan Washington, and District of Columbia Department of Health. During the event, two WomenHeart Champions will be educating attendees about high blood pressure, heart disease, and heart health.
- West Virginia Bureau for Public Health - The West Virginia Bureau for Public Health's Division of Health Promotion and Chronic Disease will raise awareness of the event and high blood pressure by sending an email to West Virginia Department of Health and Human Resources staff, including National Day of Action and Million Hearts® information on its website, and partnering with other state organizations to raise awareness and promote action. Efforts will be focused towards: community members/patients, FQHCs, rural health clinics, public health nurses, and other organizations across West Virginia.
- Wyoming Department of Health (WY) - The Wyoming Department of Health will join the National Day of Action by raising awareness of high blood pressure in local media outlets.
- YMCA of Metropolitan DC (DC) - The YMCA of Metropolitan DC will be participating in the Roll Up Your Sleeves! Health Expo on Saturday, May 9 in Washington, DC, presented by Measure Up/Pressure Down® in partnership with United Health Foundation, YMCA of Metropolitan Washington, and District of Columbia Department of Health. The YMCA of Metropolitan DC also provided event space and programming.
